The purpose of this document is to:

  • provide clear guidance for individuals, families, professionals and partner organisations regarding processes relating to Safeguarding Adults Reviews (SARs)
  • ensure that local practice is in line with the Care Act 2014 statutory requirements for the Cumbria Safeguarding Adults Board (CSAB) to undertake SARs
  • provide a framework that enables SARs to be undertaken in a proportionate way with the primary aim of multi-agency learning
  • recognise that there are other forms of statutory reviews (such as domestic homicide reviews, mental health homicide reviews, MAPPA reviews, children’s serious case reviews) and the importance of managing the interface between these and a SAR
  • recognise that the adult and/or their family must always be offered the opportunity to contribute to the review process and are provided with the appropriate support to do so. This may include involving a Care Act advocate.

    SAR Methodology Toolkit

    This toolkit outlines a range of methodologies to consider and adopt for the commissioning and delivery of Safeguarding Adult Reviews (SARs).  Statutory guidance (14.164) states; 

    The SAB should be primarily concerned with weighing up what type of ‘review’ process will promote effective learning and improvement action to prevent future deaths or serious harm occurring.  Where a referral for SAR has been agreed as meeting the criteria (Care Act 2015 s 44), the SAR sub-group will select the appropriate methodology which is proportionate for the case under review taking account of the circumstances and other factors.
